CMS Requirements

Face-to-face encounter requirements

The Affordable Care Act requires a face-to-face encounter between the certifying physician (or allowed non-physician practitioner) and the patient within 90 days before or 30 days after the start of home health care.

The encounter must be related to the primary reason for home health and documented in the medical record. Kassy Health provides a one-page F2F documentation template that meets CMS requirements. We pre-populate it from your referral and return it for signature — you do not need to create documentation from scratch.

Allowed non-physician practitioners who may certify include nurse practitioners, clinical nurse specialists, certified nurse midwives, and physician assistants who are working in collaboration with the certifying physician.
